Health Careers Scholarship Program 2023-2024

The Fort HealthCare Partners, Fort HealthCare Medical Staff and Fort Memorial Hospital Foundation offer the Health Careers Scholarship Program. For the 2023-24 academic year, Fort HealthCare is awarding over $25,000 scholarships to students seeking professions in healthcare. The scholarship is being provided for students studying at a US or Canadian-accredited college/university. Health Careers Education
Scholarships are given to individuals who already have a degree or certification in the healthcare sector and are presently enrolled in a program that will lead to an advanced degree in that field and to graduating high school students pursuing a career in the healthcare field.

Health Careers Scholarship Program Eligibility Requirement 2023-2024

  • Applicants must be residents from Cambridge, Fort Atkinson, Helenville, Jefferson, Johnson Creek, Lake Mills, Palmyra, Sullivan, or Whitewater, or employees at Fort HealthCare.
  • Applicants must be high school graduates of any accredited institution of Cambridge, Fort Atkinson, Helenville, Jefferson, Johnson Creek, Lake Mills, Palmyra, Sullivan, or Whitewater.
  • Applicants must be Medical and dental students who have completed their first professional year
  • Students in pharmacy, occupational therapy, physical therapy, speech therapy, and medical technology who have completed at least two years of study, with preference given to those pursuing masters or doctoral degrees.
  • Applicants must be Students studying to become registered nurses or nurse practitioners

Health Careers Scholarship Program Application Procedure 2023-2024

  • Forward email application requests via: health-careers-director@iokds.org
  • Application requests must be submitted From November 1 through March 1
  • Complete and submit the application form
  •  Submit A short essay (one page) describing: “What are your purpose and goals for a career in healthcare?,
    Describe the life experiences and volunteer activities that have helped shape your goals, What impact would a scholarship have on you?’
  • Submit a Transcript of recent grades covering a minimum of the last 3 semesters.
  • Submit a Resume of community involvement and extra-curricular activities.
  • Attach ACT and/or SAT scores if taken
  • Two (2) letters of reference including one from someone not affiliated with your school and any additional scholarship-specific documents required.
  • The applications must be postmarked no later than April 1.
